In collaboration with local primary care providers, I implemented the first care transitions team in Mississippi, "Health Connect," resulting in a 7% decrease of readmissions in one year.
In 2014, efforts to decrease high utilizers in the Emergency Department and readmissions were published in the Generations Journal of American Society on Aging: "A Community-Based Approach to Address Health Disparities Among African American Medicare Beneficiaries in Mississippi."
