I have served as Chief Nursing and Operations Officer, Director of Care Coordination of a Hospital System, with extensive experience in Utilization Management while providing Value-based Care and Population Health to improve patient outcomes and prevent progression of disease. My expertise includes thorough knowledge of Joint Commission and Centers of Medicare and Medicaid Regulations.
I have served on Corporate Patient Safety Committees and oversight of compliance and reporting of quality and safety scorecards for 71 facilities with Health Management Associates. In collaboration with local primary care providers, I implemented the first care transitions team in Mississippi, "Health Connect," resulting in a 7% decrease of readmissions in one year.
In 2014, efforts to decrease high utilizers in the Emergency Department and readmissions were published in the Generations Journal of American Society on Aging: "A Community-Based Approach to Address Health Disparities Among African American Medicare Beneficiaries in Mississippi."
